This will vary depending on the diagnosis.
If your child has a diagnosed medical condition, you must provide us with (prior to attending):
A copy of the medical Action Plan prepared and signed by their GP (if there is one in place);
Any reports or information about their medical condition.
Once we are notified of a child who has a Medical Action Plan, the family must fill out a Risk Minimisation and Communication Plan in conjunction with the service, that details important information we must know to be able to support your child’s health and wellbeing. The Risk Minimisation and Communication Plan is a Regulatory requirement and a required document under the National Law and Regulations. If your child requires medication, you must provide us with (prior to attending):
In date medication, in it's correct box, with a pharmacy label
A completed Medical Authorization Form
If your child has a disability diagnosis (such as ADHD, ASD etc) You are not required to provide us with anything, however - we have the opportunity to apply for additional funding to support Professional Development, resources and small group support. To appy for funding we require:
A completed KU children's consent form
A completed QLD DESE consent form
A formal letter of diagnosis, health card etc.
EPOSHC refers to the QLD 'Time Out' guidelines on exclusion periods and requirements. The goal of exclusion is to reduce the spread of infectious diseases. By minimizing contact between individuals who have an infectious disease and those who are at risk of catching it, we decrease the likelihood of the disease spreading.
Excluding ill children, educators, and other staff is an effective method to limit the spread of infections in education and care services.
